Make one affidavit, go to your local MCD office with the handwritten BC issued by the hospital at the time of your birth, file the case that you want computerized BC with name endorsed on that,fill up the form there is small amount of fees and after 10- 15 days you will get the computerized BC.
The process I have followed to get it mine from Delhi.

It is a really tricky situation. One of the requirements of passing the completeness check of CIC is that all places where signatures were supposed to be appended have been signed. Since IMM5406E (Addt. Family Info) has 3 places for signatures (2 are depending on your answer) hence if you have no children the current guidelines and form ask you to sign the declaration.

However, since you have signed the form at the main place, it MIGHT be accepted and you could be later asked to sign in the presence of a CIC official the declaration of no children. This is only a GUESS.

You are absolutely right, I had applied in the same zone as my hospital is. Though not staying in the same zone now.
You have to go to the same MCD or SDM zone, better go to MCD, they have big team there as compare to SDM office.

Does it mean that CIC is currently reviewing document received on or after June 17th ? Not sure whether they are reviewing document based on the datetime irrespective of NOC code or they sort files based on NOC code and then review one by one based on date? Anyone know whats the logic behind this

NEED URGENT HELP!!!

I am 24 July applicant for NOC 0211. I came to know that date of birth mentioned on WES report is wrong (date is incorrect, month and year are correct). I posted on this forum for suggestions and one of the members suggested to email WES about their mistake and ask them to manage the issue at their end. Once I receive the response forward it to CIC.

But it's been over a week since I contacted WES but their is no response from their side. It's a mistake from their side which might affect my application. Should I wait more or inform CIC?

Pls reply admins if not travelled out of country is it compulsary to fill travel form my agent denies any one who has not filled that form got per??????????????

Just fill ur details and mention "NA or NOT APPLICABLE" just to be on safer side. Don't take any risk by not sending this most common form.

You should immediately take a fresh report from WES with correct Name and Date of birth. And then resend your application to CIS with a cover letter, stating to ignore the previously sent WES report since it had error.
