But in document checklist they need legal document for name change. What should I provide for that?
sharmausct, you have too much questions for someone to answer them. If any of those questions would be easy, I'm sure you'd already answer them yourself.
But ok, I'll try to answer a few.

1a) Any paper I guess, but I'd just stick to a4.
1b) Some say plain copies, some say every copy has to be notarized. Precisely - in the detailed online guide it says that anything that is TRANSLATED to English has to have a notarized copy of original but the checklist documents doesn't ask for that. I did NOT notarize everything, but did most of them.
2) I did that by printing on a simple blank a4 paper, with short title on it saying which form and question it is about and I signed it and dated at the bottom.
3) I believe you can add data manually, by hand. At least - you can do it on forms that are not electronically validated (like the generic application form)
4) I'm pretty sure they will be able to. Although with ccards you never know. Some people were complaining here because many ccards were being replaced right about these days in India, because of the lack of chips or something like that. You'll know more about it than me.

I had translations from 2 separate court translators, one had it all nicely wrapped in a separate blank sheet of paper, pulled some string through it, put a wax seal, gave a separate declaration that he's certified etc, while the other just add a notice on the same A4 paper that she is a certified translator (sometimes that notice being on the front side of the paper, sometimes the back). I think they are both fine (provided that CIC guys ever see the attached notice for this other translator :D)
So, I guess that doesn't help you much in deciding. :)

I tend to submit an application for NOC 2174 but looking at the spreadsheet, I saw the total number of accepted documents is 549. I feel depressed now :(.

Apply with faith.. the sheet is a rough estimate! If you have everything ready then there is no harm in sending the file to CIC. Best of luck!

I'm still waiting for my app to get delivered. It was supposed to be there at Friday, but apparently it takes 2x more time from Montreal to Sidney than across the world, so it will arrive, hopefully, today. And I think I still have a chance, not too big though.
I strongly believe the google sheet is a overestimate. We simply can't know - even the data coming from the CIC is only an estimate.

Anyway, you decide for yourself - if you're finished with your app you know how much efforts and money you invested into it. Could you forgive yourself if you don't send it and realize eventually that you could have made it?
If you decide to send, use the courier service and hope for the best. And do it today.

Yes an affidavit from translator is required

If it is written notarized copies, it means notarized copies, if it is written original then you should send original, however you can keep all your original with you and later you may be asked to present/show the originals.

Hopefully my application can be finished within this week (till waiting for WES to send ECA to me, ETA 06/26). So my best case is this week, my worse case is early next week, and...I can't think of any worst case. Finger crossed for me please!!
