Hi vedi. Glad to help you with a few of the points here.
1. I did my medicals at Ruby Hall in Pune. The procedure took 3 hours. Eye check-up, blood/ urine samples, x-ray, physical check-up by the panel doctor.
2. I called the hospital in advance to check what all I need to bring with me. You can do the same with Fortis. Here is what I took with me.
2a. MR forms
2b. MR letter and email
2c. Medical history records
2d. Five photographs as per specifications on the CIC site
2e. Medical prescriptions from my family doc
3. After the medicals, the hospital gave me two things
3a. Original receipts of the bill payment
3b. A photocopy of the MR form - filled with my info, photo, and panel doctors signature. They kept the original with them
4. Following is what I sent to NDVO
4a. Cover Letter
4b. MR email and MR Letter
4c. Original receipts of medical bill payment (Please maintain a photocopy for your records)
4d. Photocopy of the filled-up MR form that hospital gave me (item 3b above. Please maintain a copy for your records)
4e. RPRF DD - I made it in INR from Bank of Baroda.
4f. All passports (old and new)
5. I sent the whole package through VFS. They charged me Rs. 2580 (Rs. 645 per applicant. And we are four of us on my application - total 9 passports and one PIO card).
6. Adrenaline had shared a cover letter draft, which I used for my letter as well. I am sharing it here, in case if you want to use it.
