I have the following documents for submission to Visa Office Manila, please comment:

1. Forms (I have photocopy of the forms I submitted to NS and since they are only up to March, I am adding the info for May to June)

2. Identity and Civil docs -- we have NSO birth certs for me, my spouse anf 3 y.o. daughter, CENOMAR/CEMAR for all three and NSO Marriage certs...

3.Travel docs -- passports are scanned (ok lang ba or ordinary photocopy talaga dapat?) and to be notarized within this week (nearest notary public in the area will do, right?)

4.proof of relationship -- I have NSO birth certs and NSO Marriage certs of my sister and aunties. I am waiting for their notarized PR/Citcards

5.Educ/Training -- all TORs in university sealed envelopes are ready. I plan to submit our original Diplomas and original board certificates... do I still need the cert of competion? ndi ba sapat n ung diploma bilang patunay na nakatapos ako?

6.work experience -- under 4131, I have the reference letters pero hiwa-hiwalay sila... 1 cert for the duties, 1 for the existing salary, 1 for past salary (na ako pa ang nagworkback based on my 2316file)... dun sa ibang schools na pinagturuan ko, hourly rate lang ang nakalagay plus the semesters na andun ako sa kanila. Ako na lang sana magconvert sa explanation letter kung magkano inabot nun... available naman ang contracts of employment pero ako pa ang magpapanotaryo...

under 1111, kukuha pa lang ako pero baka mahirapan ako iprove ang authenticity kc ala ako employment contract nun, although nasa 2+ years din un. salary is paid in cash, walang mandatory deductions...

7. Language Proficienccy. IELTS done. Band 7. Got 16 pts for this.

8. Settlement funds. Ok lang ba ang bank accounts na ka "OR" ko mother ko? accounts were opened year 2006 pa... I have the bank certificates plus photocopy of passbook pages with the transactions for 2009 to date.. I had them signed by the bank representive and they will be notarized this week.

9. Police certificates. NBI clearance for me and my spouse... ready.

10. Fee Payment... I plan to attach a letter stating the payment details in the letter from the CIO

11. Photos. I have 6 copies of photos for each of us. In one of the photos for each, written at the back are name, bday, and date picture was taken.

Meron pa ba ako namimiss? Meron ba ako dapat kunin sa PRC or DFA?

i think you should talk to the HRD ng hospital nyo... ask them if nag background check ung embassy then ask them wat happened, from there makakagawa ka na ng explanation... mas maganda sana kung malaman mo muna lahat bago ka mag explain sa embassy... their having doubts now, so isang mali na lang baka kung ano na gawin nila sa papers mo.... anyway, hope everything will be okay soon and the embassy will understand.. my prayers are with you...

tamas si kikokits... call the hosp and ask... ngayon, what yo can do is ask nga a lawyer to help you make an affidavit... maybe the best angle to attack this situation is to be honest that you have no intention of giving a fraudulent information but because thru the hospital, you were given the chance to be a private nurse to one of their patients... so you thought it better to use the letter head of the hosp that has given you the chance to be a private nurse... this is how i understand kasi you situation... ngayon, kung hindi ganito yuong nangyari talaga, just be as honest as you could...

BTW, what are their basis for saying your information is fraudulent? kasi don mo pwede icontest yon to clear your side...

DO I STILL DESERVE A SECOND CHANCE AFTER EXPLAINING TO THEM?

Correct me if i'm wrong, but based on this post and ur previous inputs, this is how i understand ur case ---that u have been employed as a staff nurse in a hospital where u have also been a private duty nurse on the hospital's patient (or patients?) who was also confined in that hospital and the certification was signed by the family of the patient/s using the hospital's letterhead? Did i get the info right? Further, did your private duty came right after you worked as a staff nurse for 11 mos or simultaneously?

anyway, if the above is correct, here are my inputs:
1 - Considering the time constraint for you to submit the affidavit of explanation, i agree with all those who advised here in the forum that you have to seek the help of a lawyer who has sufficient knowledge with the ins and outs of the Canadian embassy, and is somewhat familiar with the way of thinking of the visa officers.

2 - then maybe with your affidavit, you could explain that in the hospital where you work for, you were allowed to do private duty on their patients bcoz they prefer to recommend nurses who have worked or have been trained by them to do private duty on their patients, whenever the significant others of their patients seek help from the hospital when necessary.

3 - then maybe you could further explain why the letterhead of the hospital was used---that when you sought certification from the family of your patient/s who had no idea nor previous experience of issuing such certification, you committed an honest mistake of making use of the hospital's letterhead considering that you had ur duty in the same hospital, but only that ur employer is no longer the hospital but the private individual/s. Also if u could secure a new certification (to be attached in the affidavit) from the family of your patient/s (as pdn) in a plain bond paper (without any letterhead) signed by the patient or the family, stating the exact period covered and if possible supported by the patient's hospitalization records on those dates.

I hope that the foregoing could be of help. Actually, this is my first time to post an advice here in this forum, since I'm not confident yet as I don't have enough experience/knowledge in processing, as I myself have sought the help of a consultant for my own application. But if I am posed with the same problem as yours (which I hope not :(), I would do the aforementioned. (by the way, my consultant is Atty. Gil Reyes of HDR Associates Global Immigration Linkages. Immigrant visa center ang name ng consultancy firm. meron sila show every sunday sa nbn channel. I recommend them ksi mura at mabilis sila magprocess ng docs.

medyo complicated ang situation mo and we cant afford to delay the process nor commit another mistake. i guess ung problem nato needs professional action.

in my opinion lang, i don't think that declaring ur experience in an industrial clinic would do good to ur case...it might adversely affect (for me lang ha) coz u are giving FES more reason to question your documents, why you have not declared it beforehand in the documents you have submitted.

In my opinion (again) i think that the affidavit of explanation should be signed by you...
In the documents i have submitted to the VO, whenver letters of explanation are necessary for any discrepancy whatsoever, my lawyer advises that I should sign it being the principal applicant. For instance, I was not able to present a contract of employment on one of my employers, and my lawyer advised that I should make a letter or affidavit of explanation signed by me and duly-notarized.

Maybe you could also secure an affidavit of explanation from the hospital and if possible from the patient or his family, to strengthen your claim and your stand on this matter.

1. I am a second course, first course ko po Accountancy then I took up Nursing for three years. How many years of formal education po yun? 17 po ba? 6-gradeschool, 4 years for High School, 4 years for BS Accountancy and 3 years for Nursing

2. Ang Native Language po ba natin ay TAGALOG or FILIPINO?

3. SA Visa Office po natin. PHILIPPINES? MANILA, PHILIPPINES?

4. Sa level of Ecucation po? Is it BACHELOR'S DEGREE or BS Accountancy, BS Nursing?

5. Employed po ako as Staff Nurse. Sa question po about sa Current Occupation? Ano po sagot ko, EMPLOYED or STAFF NURSE?

6. I am a filipino citizen, born here in the Philippines. sa question po na Status in that Country(Philippines). CITIZEN or NATURAL BORN CITIZEN?

7. Dun sa "Additional Family Information" If my spouse is the one filling up the form, siya na po yung APPLICANT then ako po ang SPOUSE dba then his mother's and father's name? or is it the other way around?

Foremost, i prayed for you before writing my post.next, tama silang lahat consult a lawyer not an agent pero dapat immigration lawyer.among the threads parang si char ang nagsabi ng may kilala syang immigration lawyer, try mo yun.they can better help you in appealing or explaining your circumstances.you know in your heart wala kang fraud na ginawa, explain mo lahat sa lawyer at lahat ng mapaguusapan nyo that will be your affidavit.....you can even telll them you can give the contactnumbers of the person who hired you as PDN.....am hoping for the best, relax take a deep breath para makaisip ng straight...goodluck, God blesss.....
