Now the unanswered question, what about the passport pics? None were requested of her...
You take 4 to the doctor for the medical. If I remember correctly one of them gets glued to the form they give you to include with your app, the other 3 must be affixed to other places in the file they send to the embassy.
The 9 photos you send with your app are used by CIC in processing your app. Where they go I have no idea. But those are the ones you have to sign and date and all that. My wife doesn't have her Visa yet so I don't know if one of those photos ends up there or not.
You said they might need a new medical and one of the reasons is the photos are old. Then the photos to be replaced would be the 4 that you need to take to the medical. So the 9 with the app should still be ok (and you said they have not asked for replacements).
I think the main purpose of the 4 pictures for the medical and why they need to be recent is that they are the only way to confirm that the person who showed up for the medical was the actual applicant. For instance, if you had a medical condition you wanted to hide you might hire someone to take your exam. But the doctor would see the that the picture doesn't match the person he is examining and wouldn't do the exam. If you try to fool the doctor by providing photos of the hired person then they wouldn't match the photos in your application and CIC would know something was up.
Of course if in doubt there is always the CIC information line to get a definitive answer.
