Hi:

i had some issues filling out the family information in the PR documents IMM5406, my parents are divorced but its my birth mother's name on all my legal documents, even on passport. My step mother never adopted me nor she has her name in any of my legal docs. So what should i do, should i fill out my birth mother's name there or step mother, and if birth mothers name then what about her address and her marital status. Plus my i have step sister too, should i fill in her name as well in the brothers sisters column.

You have to list your legal mother, which is your birth mother. If you don't know her information, then you explain on a separate sheet.

As very clearly stated right on the forum, you must list step-siblings.
