you need to provide following docs to CIO:
1. Internship training certificate
2. A work experience certificate written on hospital letterhead; signed,dated & stamped by your supervisor/hospital in-charge/principal which clearly mentioned your duties you performed as "Intern Doctor", your salary, working hours etc.
Please note, internship certificate alone is not sufficient. you must submit work exp certifcate and your duties must match with the duties mentioned in NOC website.

Dr. Hasib, what you have mentioned is quite right.
I would like to add that I didn't have to give them a separate Internship certificate. Just one certificate that mentioned that I worked as an Internship physician from __ to___ & that my monthly salary had been ___ for that year.
I also added a paper with the duties of Internship physician in the educational hospital I have been in. The paper was a general duty one from the hospital, so I had them sign & stamp it by the hospital's seal to show that it's an official paper.
In short, you have to supply all the requirements they ask for in a work experience.

You should also try to show how similar your duties then were with the duties in the Canadian National Occupational Classification 3112 Doctor, Intern category.
