I am starting this thread for December 2018 applicants and i am one of them :)
Can the experts/seniors advise on the below questions from the application form

Q06 - If no name changes , should we just write our name select Name at birth ?
Q11 - I know we should give employers address , but just wondering about the name of contact , phone number , email address columns
Q12 - should we tick Yes for both Required to File and Taxes files ? my edibility period is from 2013 DEC-2018 DEC

Appreciate your responses in advance :)

I finally became a citizen on December 11th and received my passport today. I wanted to wait until I actually received my passport before I made my last formal post as a December 2018 applicant. Here’s the details on my ceremony:

The ceremony was remarkably fast. Most users on here seem to indicate it’s 90-120 minutes long, but mine was just under 60 minutes from start to finish. Maybe this is because Halifax has smaller ceremonies, but I’m really not sure. Anyway, my ceremony had 72 applicants who became citizens, and we were collectively from 31 different countries.

And in case anyone is interested, IRCC only collected my oath documents (perrmissions/consent and prohibitions forms) and my PR card. They did not ask for any other ID and they did not ask for my COPR, despite the fact that it is listed in the oath letter. The staff were very pleasant and the citizenship judge did a great job with the presentation.

It sounds strange, but I had to pinch myself to make sure I was awake when I was given my citizenship certificate. I applied in early 2011 for permanent residency, which was almost exactly 9 years ago. It took me nearly 9 years from initial PR application to citizen. I still didn’t quite believe it happened or that I was a citizen for real until I got my new passport today, two days before Christmas.

So ends my 9 year emigration journey to Canada, and 7 years of posting on here. Good luck to all of the remaining December 2018 applicants, and happy holidays!

Thanks for the reply. I did send it like a week ago so soon to say if they'll return it or not. (I didn't find anywhere in the form!!)

Congrats. I noticed on the sheet that your "Decision Made" was on Nov 8 but you got the oath invite on Dec 23.
A gap of 6 weeks, Can you think why this gap?
Many people have this gap but many get the oath invite
the next day of "Decision Made".

Please its very rare people get oath invite next day of decision made except for people from Scarborough who are invited for oath sometimes next day after test. Other offices take anywhere between 2-8 weeks for oath invitation from test date
