Bio-pages of passport with photo + PR Card with + Health card without photo + school record with or without photo Should be enough. Just add a cover letter explaining the situation. I did that and application was accepted and is in process.

I agree to arambi's input since seweify's nine year old child is a minor and some provinces do not provide health card with photo. Please go to your child school and ask for school records, they should be able to give you one.

Hi Guys,

My 9 years old has no photo ID except his passport biographical page (OHIP has no photo and no school card issued). Do you think it's enough? CIC site mentions a school record...any idea what that is?

What they are looking for is the evidences for presence or absence of you during the relevant residency period.

If you are in Toronto and your kids TDSB students, go to school and

1) Get the school record - ask for Index which has enrollment information with a photo of your kid. TDSB school office staff are used to and issue without any hesitation. Some times on spot but if they are busy you will have to pick up later/next day.

2) Copy of report cards - which shows the absences and late records on the top right hand side of the page. Report cards are send home usually end of every term. So you will have it ready, make copies and attach.

If you are not under TDSB look for some thing similar your school issues.

Remember, you have to attach this to PARENT'S application, not kids.

Dear Raman_Ram,
Please send your driver license and health card. In addition, attach a letter explaining that you just moved to a new province (for work reason) and you applied for new driver license and health cards with new address. A receipt or letter from your city DMV and/or from your province healthcare center will be a good reference.
Two reasons why a copy of your passport is not the required ID:
1. A copy of your biographical page of your passport has been asked somewhere else
2. I am sure that your actually passport is not delivered by Canadian gov.
I did not submitted my PR card as a prove for my ID because they already have it in their system and it will be twit to ask for it again. In summary, give them the IDs that you got from our government and CIC does not have yet.

I understand your points 1 and 2 however, you can photocopy the passport again to be reused as a part of ID (one of two pieces of ID) even though photocopy of passport has been used as a part of biographical pages. I see no harm.
CIC is also pretty much aware that they have mentioned passport as a part of two IDs as well as part of biographical pages of the passport in the Instruction Guide.

Many people have used it the same way including myself.

They want two pieces of ID for identification purpose only.

Sending a letter with explanation makes good sense.

In BC, drivers license and health card is combined together as one ID thus I sent copy of my passport along with drivers license.

For me I had no other third ID thus no other choice left because I just have passport and Drivers License (Drivers license and health card combined in one ID). My passport has my canadian address though because it was renewed in Canada.
However for people who have driver's license and health card separately, they can send both as part of two IDs.

Thank you for the detailed comment. I just found the information on the CIC website:
5. Two (2) pieces of personal identification

Both pieces of identification should show your name and date of birth, one of which must have your photo on it.

Examples include:

1. Canadian driver’s licence
2. Canadian health insurance card
3. copy of the biographical page of your passport/travel document, etc.
If there is information on both sides of your personal identification documents, provide a photocopy of both sides.

Note: Social Insurance Number (SIN) cards, bank cards and credit cards are not acceptable pieces of identification.

Happy almost New Year!

I sent in my application from Vancouver on Dec 12 and it was received Dec 16, 2014 in Sydney.

I was supposed to apply in July 2013 but with a new baby/ life/ moving / English language test/ citizenship photos, it was hard to find the time. Now I will have to apply to renew my permanent residence card because that expires in July 2015 - I heard you should apply 6 months before it expires so yea! More forms!

Sorry for late response I was a bit away from my PC.
Let me clarify something here:
1. Your son is a Canadian and has a birth certificate as a prove of citizenship so you are not applying to get his citizenship certificate. In this case, the peel district school letter will be considered as an additional document and will not affect your application process. No worries!
2. Your son is a Canadian with a birth certificate as a prove of citizenship, however, you applied to get him a citizenship certificate. In this case if you are lucky, CIC may just ask for the missing document but in most cases, CIC consider the application incomplete and return it. I hope this is not your case.
