Nikah nama both english and urdu versions. 3 copies each
Marriage registration certificate which u can obtain after your wedding from union council. You ll have to fill in the form at the union council and they will give you the certificate.
All chat logs,skype history, phone bills before and after wedding etc
Pictures of all functions including group photos with guests and relatives
All random pictures u have taken together before and after wedding at different places. Select few as they require not more than 20.
Any postal mails, courier reciepts, money transfer reciepts taken place between you and your spouse.
Rest all the docs list you may find on the official site.

Above all, your application MUST be complete, concise, correct and correlated. Make it as simple and as straight forward as you can. Do homework on your possible red flags.

Same was in my case. So I transferred sim to my cnic n then started calling again. Aftr 3 months I requested my netwek provider to give me a letter stating this is my own number and I am calling n receiving calls from this particular number since the date. Also I obtained calls record of 3 months.

dont worry..i didnt have any pre-wedding pics or gift and i didnt attach any proof of communication,,these r just supplementary things collect all ur wedding records and post wedding,,best of luck

thats what I was thinking, I will gather everything after the wedding and will attach whatever I have

Is there any special docs that she has to get prepared for the application

and is it necessary to have husbands name on the NIC?

Yes, its recommended to have your spouse's name on both NIC and Passport.
