Those who are matching their VO names, it is not the matter of VO. It is the matter of case's nature. VOs are of 2 Category. One is junior and other is senior. Simple and straightforward cases are assigned to the Junior; and complex, doubtful and ambiguous cases are assigned to the Senior one. Off course most of the cases are complex due to lesser homework and language barrier, incomplete answers and docs; so there are more number of cases in Senior officers queue. And that is the reason people who are asked for additional docs etc are in the queue of Senior VO and are being reviewed critically.

It is not always updated, it depends on the Visa officer, if it's an organized one he/she may update right away but if it's a lazy one it could take till the person reaches Canada then they update it.

No problem, yes don't ever worry too much about ECAS status. They have even made mistakes sometimes with a few people. Some members of this forum had shared their experience previously.
