I don't think you need any ID/documents to attach other then consent form if you have canadian mailing address, this is what I found from ATIP site for requirement

Proof that you have the right to make a request:

Proof may include a copy of your Canadian passport, your permanent resident card, or an official document that demonstrates you are currently living in Canada (i.e. driver's license, bill with proof of address, or work, study and/or visitor permit).
If you have a mailing address in Canada, you will NOT be required to provide proof.

Wonder why I still haven't gotten my notes then .. 35 days today.

I also didn't attach the proof on their link they says if you have canadian address then you don't need to attach this proof.

Don't worry I know many people who been waiting for over 30 days now, it seems like its taking close to 2 months these days...for me its been 30 days as well...

i did get my notes twice...nothing in it. file was transferred to isloo in jun 13. that it. got latest notes on oct 05.

u must apply notes brother
and every other cases who didn't get anything yet
must apply it to know where ur case is standing

Are there July 2012 cases getting re-med requests?
