There is no form for that you need to call to IOM if you are in Pakistan and get the appointment. So simple.

They do send a form that contains applicant's info, immigration type and a section for the physician to confirm the identity and put remarks regarding the medical results.

I received an e-mail from London a few hours ago for a re-medical and re police certificate Did anyone else from August batch receive email too?

The point was VO will send you the medical exam form and once you call IOM to book an appointment they will ask you to bring identification etc including the form that VO sends you in the email.

" Repeat medical examination; please note that medical instruction forms will be emailed separately. "

lolz now i have add a London clock on my computer to check emails when its normal working hours in London
