Section B - should we leave it blank?
Section C - I am the individual so we will put in my information

Section E - B and E are related but I am not asking for my information, I shouldn't sign it right?

Create a PDF/Word doc showing that your husband [who is the applicant] has given you consent to request this info. It needs to be signed and dated by your husband [1st time I ordered my notes I used digital sign but 2nd time I used the digital sign and they refused it after 30 days and I had to sign it by hand and send it again]
