I received GCMC notes yesterday. Notes state that my file was transferred to London on Feb 24, 2014 just like majority of the members here. I found 2 things that i should pay close attention to.
1. My Wife's mailing address, Colony name still has last spelling typo in it, but it is readable.
2. There are 2 Columns under medical. 1st Expired Medical details are Under Medical 2 Column. And below is written under Medical 1 Column.

Created Date: 2013/12/31
Updated: 2013/12/31
Type: Medical
Status: Sent
Due Date: 2014/01/30

Based on the notes above they sent me a re-medical request back in December 2013, Which my wife never received. I doubt that having 1 last spelling typo in the colony name has anything to do with it. Also under the text notes Made by Islamabad the last line says "Med's Validity: expired. New Medical sent by email" My Wife never wrote her contact email when we applied.

Members please advise how should i proceed with this. Should i start of by making London aware about the address typo and Not receiving the medical request?

Email CHC-I about it. Even if they don't reply, at the very least you will have an electronic proof that you contacted them regarding this issue. But my guess is they will get back to you in regards to this inquiry. I have sent them email twice. Once about address change and since it was a genuine reason, they replied. My 2nd email was more like whining about where my file was and since it wasn't much of an urgent matter, I never got a reply from them.

I am thinking of changing her mailing address. Can the mailing address be different then her residential address? What is CHC-I Email address? Also shouldn't i be contact London on this matter since our file is with them currently?

Yes, it can be different than residential. but providing an email address is more convenient, and fast. CIC email

Agreed. I will request to add her email address into file. What is the London Email address?

In the letter I received, London mentions that if you're in another country then they should be informed to do the similar process in another country and London should be contacted immediately in that case. Since it doesn't apply to me, I didn't really pay much attention to it but I believe her husband might not have to go back.

Even in another country they will do the same process, ask for the copy and then the original will be sent to ISB from within Pakistan only.

You can never ever send passports to another country via mail, period. Now there are 101 workarounds but no point in discussing them.

I would suggest u to involve ur mp in this case as mp will email isb and isb does respond/take action when mp is involved .. we had same prob we didn't receive our add doc req thru email although we had my wife's email add on file but we never recd the email and by chance we inquired mp about our file's progress and found out a doc req letter was sent to us thru email which we never recd .. mp emailed isb and within a day we got another copy of request letter emailed to us ...
