Need your help..

My Family Class application is processed at Abu Dhabi office. I got a call from them some days ago that they had sent me remedical forms 2 months ago and i havent done medical yet. I informed them i never got forms, and they asked me to do remedical within 30 days.

I went to doctor with Appendix C form as i dont had remedical forms and told him my situation. He did the medical and sent the results to London office. But now i noticed that he didnt put my file number on IMM 1017 form copy that he gave me and sent to London. I had given my File Number to doctor during medical and he told me he will write it.
Will this create any issue? Will London able to find my file number form my name, date of birth and nationality that is written on IMM 1017 form?

Do i need to take any action? Please let me know? Thanks

Not done medical yet, app tmrw. Sorry.

But to be on the safe side, I would contact CIC and tell them exactly what happened. They will have it on file at least.

can you please tell me what docs you will be carrying with yourself tomorrow. I have got my appointment next week, and i just got a remidical letter from the embassy, i did not get appendix C or nything else.

I would also advise you to send an e-mail to your Visa Office and explain this. I am also not sure whether doctor marks file number on it or not. JazakAllah Khair !

I saw a small error in my mailing address posted on the cic website (it was 24-4-1 instead of 24-A-1) and in order to get it fixed, I sent a fax to the CHC-ISL at 92-51-208-6900 and 92-51-208-6913 with the following information.

File #:
Telephone #:
Fax #:
Mailing Address:
Email Address:
Return Fax #:

I sent this fax last week and the address is still not changed. I did receive additional information form in April on the same address but just wanted to fix it anyways. ANY COMMENTS on how long they take to process the change of address request?

Also, following is my time. I am starting to get a bit worried given I havent got the request for re-med yet.

Applied: Feb 18 2011
Husband got approval as sponsor: March 23 2011
File Transfered to CHC-ISL: April 2011
Request for additional information: April 16 2012 (received it on April 23rd 2012)
Additional document sent: May 08 2012
Status on the CIC website: "Application received"

I guess you don't have to worry about you address. Because E- Cuse system designs to show Canadian address sense our Pakistani Addresses are in deferent formats
That's why E- Case always shows address looks like incomplete but actually is not.

please find below the price list for Re-Medical in Pakistan which i Copy from my Re-Medical Request.

For Adults:

Physical Exam = 1700 Rs.
Laboratory = 500 Rs.
X-Ray = 400 Rs.
HIV = 700 Rs.
Courier Charges = 250 Rs.

Total = 3550 Rs.

I told my wife to get a English ceritfied translation of the Nika Nama, she made some true copy and the name was spelled wrong, went a second time, again he spelled it wrong. UGH!! what do I have to deal with ><

Where should she go to make a english certified translation copy of the nika nama? She doesn't know where and it seems NO ONE KNOWS. She is in Rawalpindi.

If you don't mind me asking, where do you reside in Pakistan? As my timeline is almost similar to yours. I got additional documents request on April 23rd (Letter dated April 6th) and my status went into 'In-process' a month ago. I am still waiting for my Re-medical request.

I received a letter for re medical yesterday. It says to arrange for a medical examination, you must contact International Organization for Migration Canada Health Assessment Office at 051-111-466-472, and you should go under medical examination in next 30 days.

Q1. There is no date on the letter, what would be the deadline to get it done?

Q2. Do I have to call them on the given number or can I also go to the Doctor who initially did medical examination?

Q3. I didn't get this paragraph
“TO ENSURE FASTER SERVICE WHEN YOUR FILE IS CONCLUDED, YOU SHOULD INCLUDE A PRE-PAID PRE-ADDRESSED RETURN COURIER ENVELOPE”. (I assume its computer generated letter and has got nothing to do with me)
