Regarding your PCC, It doesn't matter what it says on your passport or ID card for addressess, because people move all the time. However, if your PCC states address you only lived until 2008 and none from 2009 -2011 then thats a problem. If you have mentioned on the forms that you lived at different addressess for more than 6 months from 2008 to 2011, and your PCC don't show those addresses then I am afraid you will have to get a PCC from your current district. For example: If you lived in FB Area from 2008 - 2009 and lived in Clifton from 2009 to present you will need a PCCs from each district.

And for the 2nd qs, I've basically been living in 2 lanes One lane till 2009, then to my parents house in another lane, then back to the first lane as thats the lane where my inlaws live too But Im getting another PCC made now,with the correction as follows:
2003-2009...House number
2009-2010...House number
2010-present...House number
