ok First of all tell ur hubby to come to Pak and get the rukhsati done asap because next they will say that we have doubt about ur relationship being genuine and will ask u to come for an interview. The interview thing will prolong ur case for 8-9 months so its better for u if ur husband comes and u do a rukhsati.No need to get worried about the forms everyone receives these forms

just gather your proofs, phn bhills pictures financial reciepts..and send them to CHC..

It would be a big plus i guess if your husband has visited you since the marriage..
