CAIPS request and IMM 5475e

I am about to order my wifes CAIPS notes and I was wondering if it is acceptable to send a faxed or scanned signed copy of form IMM5475E (Autority to Release Personal Information To a Designated Individual)?

I read on this forum that a scanned copy is acceptable. There are at least two people who said they received their CAIPS with scanned form IMM 5475E. I just ordered mine today, I hope the scanned copy will work for me.

YES, You can scan it to your spouse, let him/her sign and scan back to you, a photocopy of that one wont do any problem. I received my caips ,so no problem. to send by mail its gonna takes ages toreceive and resend plus money on that. so go ahead 8) make sure you send a photocopy of You (sponsor) pr,citizenship card,health card photocopy (2identification photocopy), No need for the sponsored person Id...

Hey how much does it cost to order CAIPS? i read somewhere in here that it cost like $5. but i search online and its like $60Cad. Also where to go to order CAIPS for cheap rates? thanks.

It cost $5 if you ask for it directly...However if you use a consultant or third party then they will charge their fees

It is only 5$. Sometimes there are third parties who can help you get the notes, maybe that is where you saw the higher rate.
You can paid by credit card or by cheque/ money order (if by check make it payable to Receiver General of Canada).
Just apply for the notes yourself, and include the consent from the person you are requesting the info for ( sponsored person)
Include your Ids as well.

If the applicant is in Canada when s/he orders the notes, it costs nothing. If this is the case, use the following form instead:
