My sitution was close to you ( not exactly) but CPP did not ask me to redo the medical, because we did not have travel histories, and I got passport request on Sept 19. 4.Your application was reviewed and we started processing on April 27, 2012.
5.Medical results have been received.
Our medicals expired...
June 24,2013 i received email from embassy manila requesting new medical certificate for my wife and 2 kids ....

I think the long wait for PR is creating undue pressure and anxiety on applicants. These forms are basic, just like any other forms and they have been simplified over the years. So you just need to fill in what the form asks for. As for CV and other additional documents, just provide as requested.
