It seems your wife is still back home and its advised to submit passports together. I think it means they didnt see your message. Have they given you 30 days to submit passports? I think you should wait for them to reply. You have sent them a message stating your change in marital status - then I would wait for their reply.

I have been waiting for them to reply on my status change email I sent last week; but then they sent a PPR. I have replied to that again.
I will now wait for a while to see if they reply.
If they don't I will have to send passport to Detroit with a cover letter stating the story.

Isnt there a phone number for Ottawa? You can try that or faxing. The reason is that lets say you do end up sending your passports, detroit isnt your processing centre its just the stamping consulate. They should then forward your case to ottawa, so it might cause a bit of delay. I think its better if you contact ottawa through fax or phone.

So I see one of the two things happening. They would pause the process and will ask me to submit my wife's CSQ. Or they will stamp my passport and will ask to sponsor my wife. First one seem more likely and I want the same as she can come right away to Canada!

This situation is no different than one getting married during the process, yours was just at a later stage. I think you might need to fill some forms, send the marriage certificate etc. Now, her background check etc. might also happen which might delay the process. I dont think they will ask you to sponsor your wife as you are not a landed PR and as of now your current marital status doesnt match your file so you might not have your passports stamped.

Summary

Effective June 1st, 2012, officers can request that an applicant's medical file be reassessed by a Medical Officer (MOF) or a Health Branch (HB) delegated staff for the issuance of a new medical certificate. The process applies to cases where a final decision of an officer is pending only due to the expiration of a medical certificate or when the validity of the medical certificate is about to expire and the applicant is unable to land before it expires. This process is limited to applicants who had a routine immigration medical examination (IME) done by a panel physician within the last 15 months in or outside Canada. The new medical certificate issued will be valid for 12 months.


So essentially their left hand applies to their right hand for an extension. Don't think it ever gets outside of the govnment offices.
