I applied online on June 17, 2013 and got an e-mail notification of approval on July 14, 2013. I think you should contact CIC.

I just got the approval message as well! Took pretty longer than expected, from June 10 to July 22. But whatever, as long as I get it. Yay! Good luck to everyone who's still waiting!!!

Question 3: A work permit with a new employer

Details of intended work in Canada:
1. a.) Other b.) Bridging open work permit (OB 485)
2. a.)Not Applicable b.) NA
3. Province: ON City: Mississauga Address: NA
4.) NA Brief discription of duties: NA
5.) Duration: I put one year from the date of application of this work permit to one year e.g. July 23, 2013 to July 22, 2014
6.) Blank
7.) Blank
8.) No

I personally attached the following documents:
1. Copy of the current work permit
2. Copies of the passport pages (first, last, any other if applicable)
3. Copy of the AOR e-mail
4. Letter stating the reason of this work permit application

PS. Points 3 and 4 are not important but I added these just to be on the safe side.

Shouldn't Question 1(a) be Open Work Permit rather then other? I guess it doesn't matter as you specified that in Q1(b).

your application considered as 3 applications. I got my BOWP first and still waiting for my dependents, but I guess, if your dependents are here in Canada with you, all of you will get them at same time and it should not affect the processing time.
