My wife's birth certificate is registered in the NSO in Manila, but with a letter missing from her mom's last name. (Her mother mispelled it at the time of birth). This was left as is for many years. Everything in my wife's lifetime of 40 plus years (school records, her first marriage certificate 20 years ago, passport, work permits for overseas - everything) matches her mothers misspelled last name.

Example....So instead of Mary Navarro Leones, my wife has been using Mary Navaro Leones, which matches her birth certificate.

In the application package for the Philippines (country specific instructions) it mentions "if there are any discrepancies in you or your family members documents" then....."submit these togeather with a sworn affidavit explaining those discrepancies".

Does my wife and her mother need to do the sworn affidavit, or just my wife? Are we going to run into massive problems with this such as having to change her birth certificate, passport and many other documents? Or will the affidavit suffice? Any advice would be greatly appreciated....

Just a suggestion, but you might want to edit your message and replace part of her name with xxxx, to protect her identity.

It's recommended to not use complete names on a public forum.
