Hi, anyone would like to help me regarding the Appendix C form that I have to take with me to my medical appointment, sa #3, visa office that will be processing the file, shall I put CPC-M or CEM , i am here in the Philippines and my husband is in Canada, i will be mailing my application to Canada once i have completed all the requirements..ano dapat ilagay sa visa office: Case Processing Center - Missisauga or Canadian Embassy Manila? Please help! Thanks in Advance!!

Hello! sa Manila po. First step send ng hubby mo sa CPC-M and lahat ng papers nyo then pag na approve na sponsor which is your husband then transfer nila ang papers nyo sa CEM. Goodluck!

I've been lurking this forum for quite some time now and I've been enjoying reading all of your experiences in waiting and waiting and waiting (did I say waiting already? ) for the any updates. I'm glad I'm not alone. Anyway, just got worried because of this..

Canadian Embassy Manila

We put CPC-M instead of CEM on our Appendix C of the Medical Report. I'm the sponsor and I've received the DM approve notice on E-CAS and the letter saying that they sent the application to CEM already. Hmmm.. I wonder if this would cause a delay. My wife hasn't received any letter/requests for additional documents from CEM. We'll wait another week for it and if it still doesn't show up, I'll shoot CEM an email about this. I just hope that little oversight won't affect the already long waiting period.

I've another question Am I supposed to send the Proof of Medical Examination described under Appendix A,Document Checklist for Immigrants/Family Class:Spouse Category; it's stated there to "Include for yourself and each of your family members Copy 2 of the Medical Report-Section A that the designated medical practitioner will sign and give to you when you undergo the medical examination"..Im goin to send the Immigration Forms & all supporting documentation (once Ive completed them) to my husband in Canada. What I'd like to know is if Nationwide Health System Inc in Makati would issue the signed copy 2 of the medical report on the day of the medical appointment or are they goin to call it in? ..Please enlighten me!!! Thanks so much in advance!!

I'm just not sure with Nationwide as I did mine at SCTS. With them they gave me the copy the same day and asked me to call after 10 days to see if there is any problem with my medicals. Usually they send it in if there is no problem but u have to call to check to, or if there is any problem in your medicals they will call u. Yes you are suppose to submit the proof of medical along with your application (you and your family members meaning if you have kids then they should go through medicals accompanying or non accompanying)

Alrighty,thanks again emrn ^_^ Im thinking abt doing my medicals with SCTS instead as i havent gotten any clear response from those who had undergone their medicals with Nationwide...are you also under FC:Spousal Category? Was there a rectal&vaginal exam during PE?
