On the left upper corner, it says Citizenship and immigration Canada with the Canada flag. That should be the logo.

As my PER letter does contain Spouse name, I am not able apply OHIP for her.

So what I am thinking that I'll ask for immigration file under privacy act and will use that as supporting document to apply OHIP for my spouse.

Hope this will work.

Can you provide more information as required for the spreadsheet so I can include your information? Once again, congrats It's good to know Sydney has hit a sweet spot in processing, a bit earlier than last year (if you recall, Sydney was processing applications so fast last year around April). And as JVJ mentioned, be ready for request for POF (if you did not include 6 months transaction already with initial application)
