I have got OHIP with PER email.
Put a cover letter explaining the situation.
Fill in this form

Send all your documents to OHIP eligibility review committee.

The eligibility review committee may send you a form so they can check your status with CIC.
After that, they will send you a letter indicating that you are eligible.
You should take that letter to service Ontario. They will issue the OHIP for you.
I think the whole process takes 3-4 weeks.
