If your official name as per birth certificate/passport has it, then yes.

Depends. IRCC sent my Sch A back because I had not filled up None in all the boxes. But not all the other forms, which is weird.
5- For the personal history, they only have a few rows which would not even cover 2-3 years of travel history, can we attach our own table with an LOE and sign it with a similar declaration at the bottom?
Yes. I attached a PDF document by copying the tabular format from the form.

Not Every one but Many ppl get Schedule A request, and it is the sign that your app is at closing stage.

yes you can write with hand . but you can use also the pdf writer if you have pdf to fill.

write the name as you have written in English

you can write NONE.

you can add another sheet. at the top write the same form name for which you are attaching. and create similar table and put all info. i think this thing is also mentioned in the form as well.

i got the same request initially .
