Document requirements
The medical examination confirmation is required upfront at the time of application and must be valid for a minimum of six months.
The applicant must include the immigration medical examination (IME) number in their e-APR.
The applicant must provide a copy of either the information printout sheet or the Upfront Medical Report form [IMM 1017B], whichever the panel physician provides upon completion of the medical examination.
Exceptions are made only in rare circumstances that are outside of the client’s control (e.g., cancellation of appointment by panel physician at last minute without time to reschedule).
When they mean one occupation (or NOC), it is with regard to the basic requirement that you have worked in one NOC for not less than 1 year (in the last 5 years). Otherwise it does not matter how many different NOCs you have worked. However for the purpose of point calculation, all your foreign work experience will be counted. Along with the above mentioned documents, you can produce the marriage certificate of your mother to prove the name change was because of marriage. You might also consider getting an affidavit made from a competent authority that says the name change was due to marriage.AnnaBG said:Hey all,

As for the marriage certificate of my mom - do I need to include both to explain her name in my birth certificate and also the name in my current application where I list my relatives?
