Congrats Sani for your medical and don't worry about that forms. just fill them up and send them. I think they have doubt on your relationship thats why they ask you these questionair. or may be they wrongly send your these forms. Its better that send them an email and tell them these forms does not belongs to my PNP category. Might be they have sent you wrongly.

Yeah MianNoman is correct may be its a mistake.. If u remember my MR they also requested me some forms and in these form they requested 2 forms that was not related to my application then i sent an email for clarification and they replied that those forms were requested in error from u...

Thank alot. Wo both should start packing now bcz waiting time is really short now.
My spouse is not canadian. He is living in spain and i have added him in my application.
So do u think that should i send an email to cic to confirm about those forms? While a lawyer in canada suggest me that fill those forms and send them while those r not belonging to my category.

I have done medicals in UAE so in Lahore, no idea.. keep calling or call Islamabad center and ask them...
i think you should send email as an urgent matter... or call LVO center...

IOM number is for appointments – Mobile No: 0306-2922575 or UAN 051-111 466-472.These numbers are too busy sani. so make sure call them early in the mornings. like between 8 or 9AM. keep ringing them. Thier office is near to Kalma Chowk, Garden Town.

Secondly if you send them an email to ask, there is no disadvantage for it. so go for that and send them an email. Bcz it possibly could be by mistake they ask you those forms.
