Hi! I am planning to get an affidavit from a lawyer in lieu of my birth certificate. Has anyone tried this before and any suggestions on which lawyer to approach in Toronto? Thanks for the help

How would an affidavit work from a lawyer in Toronto? S/he wasn't there when you were born so could s/he sign an affidavit that you were born on such and such day?
 
I have my passport and SSC (10th std) certificate to show as proof. However my last name is shown as an initial in the SSC certificate hence I am thinking of getting an affidavit. Would it not work? There is no way I can get it from India as there is no one to run around there! Please suggest if you know any alternate way. Thanks
 
I just watched the video on CIC website. It says if you don't have birth certificate, provide an affidavit.
 
Toronto, You can get it form any Lawyer, just need original docs where it shows your Dob, like SSC or birth certificate you can get it form Indian Embassy / CGI can issue based on data in your passport and you can show that layer and he can sign and seal on your letter.

For format please search on this forum and you can find very well because in the recent past many of them did follow the process.

Affidavit is an Oath taking in presence of a lawyer or a notary. You can go to any Public notary or community legal services to take the Oath and prepare the Affidavit.

You need to carry any DOB proof that you have with you (either your Passport, Secondary School Certificate, Driverse License etc).

There is one at 360 Bloor St W, Suite 207E. Mr Jeffery A Brown is the Lawyer and Notary Public Commissioner. You can get your Affidavit done there. He will charge around $20 for this service.
Ensure to book an appointment before you visit him.

Your PP is sufficient to mention your complete name on the Affidavit. No worries. You are good to get an Affadavit done.

Now that you have the template....You can print it on a legal size paper (preferably) and take it to the notary.
If you are in Toronto and not too far from Mississauga you can go to Red Seal Notary office on Thursday. The just charge $10+Tax....I guess that the cheapest you will get.

While you could proceed with going to a lawyer and getting an affidavit made, I know of several people who have gotten their parents (one or both) to get the affidavit made, stating that you are their child and born on such-and-such date at such-and-such place. I'm not sure if CIC puts more emphasis on one method or the other, but I was always under the assumption that the affidavit had to come from your parents or someone else that witnessed your birth.

Your statement is correct. However, an Affadavit is an Oath/ Solemn declaration stating the information provided in the Affadavit is true to best of their knowledge. Any individual can take an oath confirming his/her DOB.
