Address history in PCC form - BLS Canada

I am planning to apply to PCC through BLS Canada. I am going through the PCC form, and there is section for filling the address. I thought PCC from BLS is for whole country and not for the specific address.

1. Does PCC from BLS is cover entire country or bound to address?

2. Do i need to mention all address where i stayed in India, or only permanent address? In the sample PCC form they provided did not have any address filled.

3. In the form, its asking period of stay in India - I stayed there for 26 years. So, i need to mention 26 years right?

2. Do i need to mention all address where i stayed in India, or only permanent address? In the sample PCC form they provided did not have any address filled.
- Don't know about this. But again, since they don't mention it on PCC - it shouldn't matter

3. In the form, its asking period of stay in India - I stayed there for 26 years. So, i need to mention 26 years right?

Just write the years as in 19xx (birth year) - 20xx
