Correct me if im wrong. Please

Regarding Section A

Applicant = Me
Spouse/Common Law = My husband (Canadian citizen)
Mother = my mother
Father = my biological father or my step dad (my mothers current husband)

Regarding my mother she has re-married since my biological father passed away. and her surname change to my step dad's surname now.
Q> What do i put in my mothers status re-married or married, do i put in her new married name or the surname when she was still with my father? who do i put my biological father or my mothers new husband.

Section C

Im gonna put details here of my brothers and sisters. however we are 5 in the family and the box aren't enough to mention them all.

1. Can i put both my brothers on the same box since they are twins?

Thanks!

Use your mums new married name . On one of the forms it will ask for your mums maiden name . For additional information print off another form and mark it 1/2 2/2 ....... DO NOT PUT THE TWINS NAME IN BOTH BOXES.

This is hands down the most confusing form for most applicants.

For your siblings (including half and step siblings) just list as many as you can, but on the last line write "see Addendum [A, or whatever you choose] for additional family members". Attach a separate sheet of paper with their details and make sure to label it as Addendum [...]. Also write your name and DOB across the top, as well as your sponsor's name and DOB...just in case.

Otherwise, if you just say "see attached" and it somehow becomes unattached, they may not know what it is or where it belongs.

Hey,

As far as I remember, it was mentioned in the guide IMM 5289 that we need not complete this section. I will quote exact words also-

" if you are filling outhouse the application for permanent residence from within Canada-spouse or common law partner in Canada Class (IMM 5289) , do not complete this section"

I did not mention the details of my sisters and brother.

Please help me with this.....

The applicant will need to fill out the additional information sheet , the sponsor does not . I guess it could differ from different countries. Will have to let a more senior member answer that one sorry .

As for any attachments you send you should write the name of the applicant and date of birth, just incase it gets separated.

If it was in the guidelines why didn't you fill it in? No point in worrying about it now,i am sure they will ask you for it at some point.

Father = put your biological dad. Then on a separate sheet of paper, write the name of the form at the top, IMM [...], your name and birth date. Then mention you wrote your biological father's name on the IMM [...] form and that you have a step-dad then write his full name. This way, you have provided all the information.

Your mother's status should be "married" and her put her new surname. List each of your brothers separately.

Hey, what I meant was, that it was mentioned in the guide to not to fill that section c if m applying from within Canada ( please read the quote from my previous comment).
I still have a printout of the guide where I have highlighted the same.
M worried cz I don't want any delays.

I also said in my quote that it could differ in different countries. Also said let a senior member answer this question. You are applying inland.I am applying outland , so if it said in your guidelines not to fill it in there you go ,you have your answer. None of us want delays.

Hey,

I think no matter if one is applying inland or Outland, the form IMM 5406 has to be filled in and I did. Only the section C is what I left as N/a cz of the guide for inland applications (IMM 5289).

I guess all of us did right. :D

And m sorry too for the confusion.

Thank you for all the answers.

Ill just provide a new sheet for my step dad and my brother with same details as they asked. i believed this could be computerized ill just copy the format they have on the forms? and on the last box for siblings ill just put? (see etc etc)

Thanks!

Hello all,

I have a question regarding this form (IMM 5406).

First of all I want to mention that I am a Canadian Citizen (by birth), I was married in Greece and had both my children born there. I have made my children Canadian Citizens (they have their Certificate of Citizenship) and we recently moved to Toronto. I am now filling out the forms to sponsor my wife, who is Greek (born in Germany!). My wife's family is living in Greece (and has no plans to come live here). They consist of a brother (who is married with children) and her parents (mother and father).

Do i have to complete this form? I am only sponsoring my wife. My in-laws or brother-in law will most likely never come here. Perhaps only for a visit.

you as the sponsor do not fill out the form, since this is a form for applicants applying for PR. ONLY the principle applicant (and any dependants listed in the application applying for PR) need to fill out the additional family information form. Your wife's family, you and your children will need to be listed on her form.

For application purposes, you are not the "spouse" or "family member". you are ONLY the sponsor, and the sponsor does not fill out any applicant forms.
