I think he's saying left out only a page with no info filled in, only instructions. If so I doubt this would matter but I haven't checked what the form looks like.

oh, I misunderstood. I believe the last page was a checklist of all the forms required and who needs to sign each form. I guess to ensure both parties sign. I can't remember if this was included in the package or if it was for personal reference.

Actually I just opened the actual checklist form (IMM5533e) and the blank version and there is no page 'on which there is just instruction' so you're correct and I do not understand what form the poster was referring to.
