FOR EXAMPLE: 10. Authority to Release Personal Information to a Designated Individual (IMM 5476) (NOT MANDATORY)

  • It says it isn’t mandatory but if we leave the question box blank will they think we forgot to check it? It doesn’t apply to us as we are not including it.

  • As well, for the medical examination section; it says DO NOT COMPLETE at this time so do we leave the check box blank or check it? Do we write something like available upon request?

  • Or do we attach separate paper with explanation?

Even in the document checklist, as asked for Medical Examination?

I didn't write n/a beside the medical box. I just didn't tick it. They know that people are doing this when requested so they dont expect it to be in application packages.

What about for 10. Authority to Release Personal Information to a Designated Individual (IMM 5476) (NOT MANDATORY). I'm not including that form ,as it's not mandatory, should I leave the box on the checklist blank and write n/a or check it and write n/a or just leave totally blank. What did you do?

I left it blank.

You can put n/a if you want. Definitely don't check the box though.

Oh shit. For the checklist that didn’t apply to me I didn’t put anything and left it blank. Is that fine?

Probably, but safer to put N/A. Wish they'd just add a box for "N/A" (!!)
Already passed mine though. But it does say, check all that apply. It doesn't say to put N/A. I guess the N/A is just for forms, but I also forgot to put those on the forms since the forms already blocked me from typing the one's that wasn't applicable. So I guess that's fine also, right?
