If you find it hidden it means that its interview recommended. They hide it because they think it might unfair advantage to applicant. They take 30 days to provide notes as someone is deciding what to hide in notes.

I think your file might be flaged for interview. VO or reviewing officer has found some red flag in your file. Does it have any comments by Visa Officer. By now atleast one visa officer might have left detailed notes about your case. First go through and understand what red flag is in your case.

Usually document request will delay the process. Minimum is 1 month max is 6 months. If there is any kind of suspension or complex it will be kept in queue for 3 months to be reviewed by higher officer.
