ECAS will not show medical passed, it will be available in GCKey. If you are in Canada, i think you can contact your country's embassy for getting police certificate from your home country that will be easiest way.

I also gone through same process. Your file is moved to your local visa office, status changed to in process again mean local visa office just started processing from their side. Soon you will get medical request.
