    Thanks guys. Took your advice and called them. They said that I don't have to pay for my spouse and that I just have to give a letter of explanation. Apparently that's a generic letter.

    Two different name of same person in passport

    Does your name or your wife's name have initials in the other documents? In India, we mostly add initials instead of surname. But passport does not allow initials. If you have initials, I don't think you have a problem.
