    Both of you have to fill this out separately. You have to fill the form as the applicant and include your family details and ur spouse has to fill another form as the applicant including your wife's family details. Hope this helps. All the best!

    Ah about job description, I'm not so sure. I'm sorry man. My previous employer gave me a letter with the duties mentioned. Maybe the seniors in this forum will be able to guide you better with regard to getting a letter from your colleague.

    You could give a LOE, stating that your previous employer is refusing to give all the details. You could also attach your offer letter which will state that you are a permanent employee and your payslips. Some companies have a specific structure for reference letters. This happened to me as...

    Are you sure you need a birth certificate? It is required only if you have dependant children. Check this out, http://www.cic.gc.ca/english/resources/tools/perm/express/intake-complete.asp Does your personalized document checklist include a birth certificate?
