No, it's not required for self-employed application. I didn't include it in mine. Neither did any of the people I discussed with prior to sending in my application. I just sent copies of my degrees, diplomas and professional development courses.

It doesn't matter as far as I saw from my research. A few of the people I spoke to moved to a different place other than was stated on the application form. This doesn't apply to PNP though. HTH.

Yes, they accepted but when they asked for all additional documents later on, I included the business plan then because that was all submitted online. If I'd included all the additional documents they requested with the paper application, it would have been over 100 pages long.

You can apply for both at the same time and when one is finalized, you can withdraw your application for the other one.

You need to mention all sources of income including your day job. If you don't mention it and they find out, it could be classified as misrepresentation.
