You have to notify the CIC, they should be able to give you the forms to fill out to add the baby once it's born.

I too only have an associate (2 year) degree, but not a bachelor's...so I put Post Secondary and wrote in / typed in the area (associate 2 year degree). It doesn't matter too much from what I've read.

Your name may be the same but fingerprints are unique so unless you have something to hide you shouldn't worry, that's why they need to confirm (if that's even the reason).

Has to be a letter from your HR department confirming your employment, wage, employment dates, etc. If you work for a big corporation you should be able to contact your main HR department or go to the internal company HR site and print this information or request it. If you don't have a HR...

Oh...damn maybe I should have done that. I submitted mine with YYYY-MM format. Oh well, I'm sure they'll request another if it's incorrect. I uploaded it over a month ago now and haven't heard back. My PR application is in process at the visa office right now. No movement since SA.

The only work around is to write the date in YYYY-MM format, not Ongoing. I had this problem too. *edit* - I posted mine early January 2017, around the 18th of January. There was no new form at this time. Glad they updated it!

I submitted this form with my application originally. I also included my original police certificate, though, so that's partly why I included it in my packet. Anyways, right when my application was received they requested my Form 5669 Schedule A to be uploaded on MyCIC, there should be a box...
