Go with PNP, i had a similar score, 422, and lost hope of getting ITA. I applied to Saskatchewan PNP, got 600 points and waited pleasantly for a draw with my new score of +1000 points.

I was in your exact same situation. I do not know if you can add a new family member post ITA. I did not try that because i knew that it was going to be very difficult to get the passport for my new baby in time. You can give it a try if you think you can get his passport on time.
About medicals... you will get a medical report the day you do your medicals. You have to upload that on your application as proof. The results are submitted by the clinic directly to cic.
You can add your new baby via a Case specific enquiry (CSE) after submission of your application.
