Can anyone please tell me there is a question in generic form " do you held any govt position" ? I clicked yes . Do i need to provide proof or not?

So if someone has a govt. position , that person has to click yes in govt. position question. Does that person has to provide nay copy of govt id. just for clarification?

The generic application form asks if she's ever used any other names. I'd put the other name there. Does she have proof of using both names throughout her life? for nick name , you don't have to provide it. but if it is different in the docs , then you can have an affdvt
