Why don't you ask your boss to modify reference letter ? This is just a game with putting different words together, your boss may not care about what exactly to write in this letter

If you are worrying, resend it, and say that in the previous email (put which date it was sent) you didn't present your name and last name in the required format... Just to be sure that they received so important document, you are resending this email again, with your name in a right format

My official title was postdoctoral fellow, my work permit said that I am software developer. I chose what is written in a work permit. The main thing - reference letter should correspond to NOC that you specified in your application

I came at the next day and asked doctor, and doctor answered me that my blood, urine and x-ray results are fine. But he didn't show me examination form which goes to Ottawa (in general, they don't show it)

Try GCMS notes! Finally, you can send email to CPP-Ottawa, explaining that your processing time is very above then average, and you are worrying, if you have lost their emails (which were considered as a spam), or something else

I think you are worrying to much. They will want to speak with your boss, not with some random manager.
