u can extend ur visa even before 90 days . My older study permit expires in april 2014 but i extended it and got a new one this month only .

Usually the visa officer will say it is a course mismatch and will reject the visa. So to be on a safe side , take admission in a course related to non medical stream and change your course once your reach Canada.
