You can still use the old forms to submit up until January 31 2017 if your application is ready to go.

New and old forms are being accepted now until January 31, 2017. After that, only new forms will be accepted.

Copy is fine. Option C is available on CRA site on left hand side when you login (just expanded version of Notice of Assessment)

If you live in Canada, check No, and it'll gray out the rest of the boxes relating to that question meaning that you don't have to answer it. IMM 1344 also has a Validate function too in Adobe which will ensure that you've filled out all the required fields before the barcodes are generated.
