Do u already have a letter with all these duties listed? If not, choose one noc close to ur profile and prepare the letter based on the noc duties - just follow the reverse order, that's it. Ask ur employer to put the key points in the reference letter

I seriously doubt whether there is a standard format for this email? Some say they received reference number with medical and others say they just got a reference number in AOR I guess that is case by case basis. CIC guide says following in the process - what u received is an acknowledgement...
