Hi, 5669 is for PA. You'll be attaching it to your account later when you're given instructions to do so. You can get to choose to hand sign it and send through courier or just type your name and it'll serve as your signature. The instructions for that form is found in CIC website. :)

Hello, question. In IMM 5532 part A #5: addresses where the sponsor have lived in 5 years time. Did any of you include the addresses when the sponsor travelled to visit you in months? Or did you just write the sponsor's addresses where he/she is a resident?
