I put N/A for these. For form 5481 - if it's only you and your wife, do you put N/A or 0 in the boxes in question 5B onwards, as I think 5A is meant to be '1'.

Question about photocopies - for documents like copy of my passport, marriage certificate, etc. when it says to send a photocopy, does it mean literally just a photocopy, or does it need to be notarized/certified etc. Keep in mind all our documents are in English.
