'Declaration Form' on Part 3: Immigrants Guide

Pages 10 and 11 on the Middle East version of Part 3: Immigrants Guide are Declaration Page forms.

They don't seem to be connected to any other pages in the document.

Does anyone know who needs to fill this out?

I am sponsoring my wife (we have no kids and no previous marriages).

Does this apply to us?

I imagine the following forms definitely need to be filled out which come immediately after: Supplementary Information Form, Education and Employment History and Military Service?

Page 10 and 11 don't apply to you.
The form after that does need to be filled out. The Supplementary Information Form has 4 pages: Travel, Education and Employment, and Military.

Thank you. Out of curiosity, who does that Declaration Form apply to, I just couldn't connect it to preceding forms.

Also, the last form is the Residency Questionnaire - FC3. It says it's 'to be completed by the Sponsor who is the child's parent'

Since we have no kids and I'm just sponsoring my wife, we simply don't send this one across I assume?
