Mine doesn't say Option C Printout either, just Income Tax Return Information-Regular. I think that is the right one because I ordered Option C and that's what they sent me.

I've been using Adobe Acrobat Reader dc and Foxit Phantom PDF, both of them are latest versions. It's the same, two pages of barcodes without any signature boxes (page 4 and 5) and the last page with consent and declaration with two places for signature. But thanks for the suggestion.
